前路减压植骨内固定治疗胸腰椎骨折临床疗效分析

目的探讨前路减压植骨内固定治疗胸腰椎骨折的疗效,分析前路手术优缺点。方法我院于2001年6月至2005年6月,采用前路减压加椎间植骨钢板内固定治疗胸腰椎骨折34例,随访观察其功能效果,分析前路手术的并发症。结果本组所有病例脊髓均获得有效减压,术后获随访12-68个月,平均16.5个月。术后出现的神经功能改善情况：21例不全截瘫患者中,19例有Ⅰ-Ⅲ级恢复;2例全瘫患者,未有任何恢复;13例无神经损害者,术后功能良好。随访X线见植骨均已融合,无神经、血管、输尿管损伤并发症。结论前路减压植骨内固定术减压彻底,可重建脊柱的稳定性,神经功能恢复良好,是治疗胸腰椎骨折的一种有效的方法,但是要注意防范并发症。

Clinical analysis of internal fixation by anterior approach and bone graft in the treatment of thoracolumbar spinal fractures

Objective To evaluate the clinical effects of internal fixation by anterior approach and bone graft in the treatment of thoracolumbar spinal fractures.Methods From June 2001 to June 2005,a series of 34 cases of thoracolumbar spinal fractures adopt internal fixation by anterior decompression and autoallergiciliac bone graft.Results The durations of follow-upranged from 12 months to 68 months(average of 16.5 months).19 of the 21 patients with preoperative impaired myelonic function also gained satisfaction.Their nerval functional recovery achieved 1 to 3 degree after operation.Other 13 patients without nervous lesion recovered fully after operation.Conclusion The method of anterior decompression and internal fixation is an effective approach to treat thoracolumbar spinal fractures,which can decompress the vertebral canal,rebuild spinal stability,and revert spinal function.Meanwhile,it's necessary to take measures to prevent complications.
