慢性阻塞性肺疾病继发急性呼吸衰竭的机械通气治疗

通过对８例慢性阻塞性肺疾病继发急性呼吸衰竭患者机械通气及撤机的临床实践，探讨了ＣＯＰＤ病人机械通气的治疗及其撤机的方法和技术，以及撤机成功所需达到的呼吸生理指标，认为ＣＯＰＤ病人撤机中最重要的是确保试脱机前ＰａＣＯ２等呼吸生理指标接近病人缓解期水平。

Weaning from mechanical ventilation in acute respiratory failure of patients with chronical obstructive pulmonary disease

The severer COPD patients with acute respiratory failure often dependent upon ventilator in various extent when they are treated with mechanicalventilation. What clinicians focus on is how to get these patients off the ventilator successfully. Based on our clinical weaning practice for 8 patients, we proposed the PSV (pressure support ventilation) technique the respiratory physiology parameters and the index of the weaning. We considered that the most importantin weaning patients with COPD is to ensure that the PaCO2 level prior to weaning trial must be near the of the baseline lever of PaCO2.
