保留迷走神经干的下咽癌及颈段食管癌切除术术式探讨

目的：通过对下咽癌、颈段食管癌病人行保留迷走神经干的喉及食管全切除后,以结肠代食管的术式,探讨保留迷走神经干的可行性,及保留消化道生理功能的优越性。方法：10例下咽癌颈段食管癌病人行保留迷走神经干及腹腔各分支,连同胃留在腹腔内的喉全切除和食管切除,以结肠代食管;另5例病人行喉及食管全切除,不保留迷走神经,以胃代食管,比较两组病人的消化道功能。结果：保留组术后腹胀、腹泻、返流及相关生理指标明显好于不保留级。结论：下咽癌及颈段食管瘤病人在做喉及食管全切除时保留迷走神经干是可行的,有利于患者术后消化功能的保护,提高病人的术后生活质量。

Comparison study of remain vagus nerve or not in the esophago-colon-gastrostomosis on patients with esophagopharyngeal carcinoma

Objective:To study the surgical procedure which remaining vagus nerve or not in the esophago-colon-gastrostomosis on patients with esophagopharyngeal carcinoma. Methods:Ten patients with esophagopharyngeal carcinoma underwent surgery of esophago-colon-gastrostomosis after esophagectomy which remained vagus nerve, andfive patients did not remain vagus nerve in the gostroenterostomy. Te time of gastraldrainage,hydrochloric acid in gastric juice and serum gostrin were determined befor and after surgery. Results:The digestive function was better in the group of remaining vagusnerve than that of not remaining vagus nerve oroup. Conclusion: The procedure of remaining vagus nerve for patients with esophagopharyngeal carcinoma had protected the digestive functions and improved life quality of patients.
