2型糖尿病患者血C反应蛋白与糖尿病肾病相关性分析

目的观察血C-反应蛋白（CRP）在2型糖尿病肾病患者不同时期的变化，探讨CRP的检测在预防和诊治2型糖尿病肾病中的应用意义。方法116例2型糖尿病患者按尿白蛋白排泄率和肌酐水平分为正常尿蛋白组30例，微量白蛋白尿组37例，大量尿蛋白组24例和终末期肾衰组25例，并设健康对照组40例。测量各组CRP，并应用统计学方法比较2型糖尿病各组与正常对照组CRP的差异。结果2型糖尿病各组血清CRP水平较对照组明显升高，且其水平随蛋白排泄率的升高而升高。相关分析显示，CRP与病程（r=0．606，P〈0．01）、UAER（r=0．671，P〈0．01）、甘油三酯（TG）（r=0．405，P〈0．05）、肌酐（Cr）（r=0．324，P〈0．05）呈显著正相关。结论CRP与糖尿病肾病有关，且与糖尿病肾病的程度相一致。

Correlation between C - reactive protein and diabetic nephropathy on patients with type 2 diabetes

Objective Observe the C -reactive protein （CRP） level on different stages of patients with type 2 diabetic nephropathy,so as to apply the detection of C - reactive protein （CRP） to prevent,diagnose and cure type 2 diabetic nephropathy. Methods 116 patients with type 2 diabetes were divided into normal - albuminuria group（30）, micro - albuminuria group （ 37 ）, massive - albuminuria group （24） and terminal stage renal failure group （ 25 ） according to urinary albumin excretion rate, with negative control group （40）. C - reactive protein was measured and statistically analyzed. Results C - reactive protein levels of all groups of type 2 diabetic nephropathy are higher than that of negative control group, ascending with albumin excretion rate. There is significant positive correlation between C - reactive protein level and course of disease （ r = 0.606, P 〈 0.01 ）, urinary albumin excretion rate （UAER） （ r = 0.671, P 〈 0.01 ）, triglyceride （TG） （ r = 0. 405, P 〈 0.05 ） and creatinine （ Cr ） （ r = 0. 324,P 〈 0.05）. Conduslon C - reactive protein level is positively relative with the degree of diabetic nephropathy.
