Evaluation of blood-brain barrier integrity and collateral circulation in patients with ischemic stroke by multi-mode CT

Objective To analyze the value of multimodal CT in evaluating blood-brain barrier integrity and collateral circulation in patients with ischemic stroke.Methods A retrospective analysis was conducted on 76 patients with confirmed ischemic stroke treated in our hospital from June 2018 to December 2019,who underwent multimodal CT examination,including CT plain scan(NCCT),CT perfusion imaging(CTP)and CT angiography(CTA).Distribution of perfusion range was analyzed and region of interest(ROI)was drawn to determine perfusion indexes such as cerebral blood flow(CBF),cerebral blood volume(CBV),mean transit time(MTT)and surface permeability(PS).Results 1)According to collateral circulation grouping,CBV,CBF and MTT in the good group were significantly higher than those in the bad group,while,significantly lower than those in the bad group(P<0.05).The infarct size and mRS score at the last follow-up in the good group were significantly lower than those in the poor group(P<0.05);2)According to the grouping of hemorrhagic transformation,the PS of HT group was significantly higher than that of non-HT group,while the MTT was significantly lower than that of HT group,with significant differences between the two groups(P<0.05).Conclusion For the establishment of cerebrovascular stenosis and collateral circulation,multimodal CT can achieve good imaging effect,which can observe the establishment or opening of collateral circulation,and then predict the long-term prognosis level of ischemic stroke patients.In addition,multimodal CT can effectively assess the integrity of BBB and the risk of HT,which is of great clinical significance for the limitation of thrombolytic therapy and the avoidance of complications.
