多功能足新型矫形外固定器的临床应用与效果

目的:报道一种多功能足新型矫形外固定器的构成、临床应用及效果。方法:笔者研制一种多功能足新型矫形外固定器,由3对、6根金属杆组成,每根金属杆具有单独的延长、加压功能。利用固定在胫骨、跟骨、跖骨共4根克氏针,将小腿、足部两侧成三角形连接为一体,进行足三维矫形,必要时将固定杆延展至股骨。2004年一月以来,使用该外固定器治疗不同年龄、各种原因引起的马蹄内翻足畸形,先天性胫骨假关节,脑瘫双侧瘫,结合肢体延长杆进行肢体同步延长,治疗双下肢不等长,双下肢同时延长进行身材增高;去除足横杆、斜拉杆治疗下肢股骨、胫骨骨折,3年来共计180例,效果良好。此外,还治疗膝内翻、膝外翻、踝内翻、踝外翻。结论:多功能足新型矫形外固定器治疗马蹄内翻足畸形,先天性胫骨假关节,脑瘫痉挛性双侧瘫,与肢体延长器合用可使肢体同步延长,治疗双下肢不等长和实施身材增高,去除足横杆、斜拉杆可治疗股骨、胫骨骨折、膝内翻、膝外翻、踝内翻、踝外翻。3年来临床病例证明该外固定安全、可靠、有效,用途广;与Illizarov环形外固定器相比,患者更舒适,轻便;其结构合理,制造成本低、价格便宜;适合国内有条件的医疗单位广泛应用。

The Application and Effect of A New External Frame on Foot with Multiple Functions

Objective: To report the construction and clinicle application of a new external frame on foot with multiple functions. Method: The new external frame on foot is consisted of 3 couples with 6 metal rods. The rods have lengthening or compressing force on foot and leg in three dimensions respectively through 4 Kire wires in tibia, calcaneus and tarsals. 180 cases of club foot, tibial pseudarthrodesis, cerebral palsy, limb lengthening combined with lenthening rods and fracture of lower limb without rods on foot were treated by the external frame since Fabruary 2004, and cilical results are very good. Conclusions: The new type of external frame designed by prof. Wang han lin is safe, effective and reliable for the treatment of club foot in three dimensions, tibial pseudarthrodesis,cerebral palsy, limb lengthening and fracture of lower limb.Besides, It is also effective for connection of genu varum, genu valgum, varus ankle and valgrs ankle. Compared to Illizarov external frame it is more simple, comfortable, cheep and easilky to be managed.
