Research progress in pathogenesis,diagnosis and treatment of subacromial impingement syndrome

Subacromial impingement syndrome(SAIS)is a common disease in shoulder diseases,and its overall understanding is still insufficient.Over the years,many hypotheses have been proposed to describe the pathogenesis of subacromial impingement syndrome,but no clear explanation has been found.In recent years,the endogenous mechanism of denying the existence of impact has been more and more recognized by the public;The physical examination specificity of several different subacromial impingement syndrome is low.Multiple randomized controlled experiments show that the surgical treatment of subacromial impingement syndrome has no better effect than the conservative treatment.At present,subacromial plasty/decompression is widely adopted in China,because there is no direct relationship between the degree of rotator cuff injury and pain,In terms of treatment,we should focus on the role of inflammatory cytokines in joints.Therefore,we need to make a new definition of“impingement”theory,and then change the treatment concept of SAIS,from surgical treatment to conservative treatment.
