2型糖尿病患者血清C肽水平与神经传导速度的关系

摘    要:目的探讨2型糖尿病(T2DM)患者血清C肽(C—P)水平与神经传导速度(NCV)的关系及临床意义。方法选取128例T2DM患者,根据NCV情况将其分为两组:NCV正常组76例,NCV异常组52例。另设40名健康人作为对照组。分别检测各组的病程、体重指数(BMI)、糖化血红蛋白(HbA1C)、空腹血糖(FPG)、餐后2h血糖(2hPG)、空腹C肽(VC-P)及餐后2hC肽(2hC-P)值。结果与NCV正常组相比,NCV异常组病程明显增长(P〈0.01)、2hPG明显升高(P〈O.01)、FC—P和2hC—P水平均明显降低(P〈0.01);T2DM患者的HbA1C、2hPG、2hC—P水平与对照组相比差异均有统计学意义(P〈0.01)。NCV与病程(t=10.329,P〈0.01)、2hPG(t=8.715,P〈0.01)呈负相关,与FC—P(t=2.402,P〈0.05)、2hC.P(t=19.696,P〈0.01)呈正相关,尤其与2hC-P密切相关。结论血清C肽水平与T2DM患者的神经功能密切相关,C肽对神经具有保护性作用。

Abstract:Objective To investigate the relationgship between the serum C peptide level and nerve conduction velocity in type 2 diabetes mellitus. Methods The study included 128 patients with T2DM and 40 healthy controls. The patients with T2DM were further classified into two groups of normal NCV (76 cases) and abnormal NCV (52 cases). The duration of diabetes, BMI, HbAlc, fasting blood glucose (FPG), postprandial 2 h blood glucose (2 h PG), fasting C-peptide (FC-P) and postprandial 2 h C-peptide (2 h C-P)were measured. Results Between the patients of normal NCV group and abnormal NCV group, diffences existed in duration of diabetes (P〈0.01), 2 h PG(P〈0.01)and C-P(P〈0.01), especially more significantly in 2 h C-P. The differences of HbAlc, PG and C-P between two groups of patients and healthy controls were significant(all P〈0.01 ). NCV was negetively correlated with duration of diabetes(t=10.329, P〈0.01 ), 2 h PG (t=8.715, P〈 0.01 ), but was positively correlated with FC-P(t=2.402,P〈0.05), 2 h C-P(t=19.696,P〈0.01), especially tightly correlated with 2 h C-P. Conclusion The level of serum C-peptide was tightly correlated with the neuropathy in T2DM. C-peptide may have protective effect on neuropathy.
