核素全身骨显象对鼻咽癌骨移转的探讨

对本科将近１０年间所做全身骨显象２３３３例中的３４例鼻咽癌患者的全身骨象结果进行了分析，并结合复习文献，在鼻咽癌骨转移的发生率，分布特点及与颈部淋巴结转移和颅底骨破坏的关系等方面进行了讨论。其中１５例骨显象为异常，结合临床及Ｘ线检查确诊为骨转移，其发生产达检病人的４４．１％。骨转移患者全部为多发骨转移，平均每例病灶数８．７个，骨转移的部位以脊柱和胸部为最多，其次为骨盆，肢体，颅骨，肢体转移中以下肢

STUDY ON THE BODY NE SCIENTIGRAPHY FOR METASTASES IN CARCANOMA OF NASOPHARYNX

The results of whole body bone scan in 34 among 2333 cases with confirmed carcinoina of nasopharynx were analyzed within 10 years.The relationship between incidence,distribution of bone metastases in nasopharyngeal carcinoma,and the metasmses of lymph nodes in neck area and destruction of skull base were discused.Results showed that bone scientigraphy in 15 cases were abnormal.The incidence of bone metastases compared with clinical data and X-ray examination were about 44.1%The patients with bone metastases all have a multiple metastases with 8.7 le-sions per case.The site of bone matastases was more common in the spine,ribs,follo-wing orders were pelvis, Limbs and skull. Metastases of limbs was more predominant in lower limbs( 8.9%rather than upper limbs(2.3%).The results also revealved that there was a correlation between the bone metastases in nasopharyngeal carcinoma and the destruction of skull bases(P<0.05).That is the incidence of bone metastases with the diagnosed destruction of skull bases was higher than those patients with no skull destruction.A1-thouhgh the incidence of metastases to the lymph node in the neck area in the patients with bone metastases was higher than those with no bone metastas-ases;however,there was no statistieal significance between them,further study was required.The authors realize that the spreaed of masopharyngeal carcinoma is prim-arily via the lymphatic channels to the cervical chains of lymph nodes,and then invade the internal jugular veins resulted in blood-borne metastases. The way of distant see-ding of nasopharyngeal carcinoma to the skeleton does not differ much from other Carcinomas.
