克氏针结合外固定支架与锁定加压钢板对桡骨远端复杂关节内骨折关节功能恢复比较

目的探讨克氏针结合外固定支架与锁定加压钢板对桡骨远端复杂关节内骨折的疗效。方法78例桡骨远端复杂关节内骨折患者按照手术方式不同分为克氏针组(42例)和钢板组(36例)。克氏针组采用克氏针结合外固定支架,钢板组采用锁定加压钢板固定治疗。比较两组患者骨密度变化、骨折愈合、腕关节功能恢复情况及并发症发生率。结果克氏针组与钢板组愈合率、骨折愈合时间、桡骨缩短程度、骨密度比较差异无统计学意义(P0.05)。C1、C2型患者:克氏针组、钢板组手术前后各时段腕关节功能评分比较差异均无统计学意义(P0.05);C3型患者:克氏针组腕关节功能评分明显低于钢板组,两组间比较差异具有统计学意义(P0.05)。克氏针组和钢板组并发症发生率比较差异无统计学意义(P0.05)。结论克氏针结合外固定支架治疗C3型骨折患者疗效明显优于锁定加压钢板。

Comparison of joint function of external fixator combined with Kirschner wires and locking compression plate in patients with intra-articular fractures of distal radius

Objective To explore the clinical efficiency of external fixator combined with Kirschner wire and locking compression plate in treating patients with complicated intra-articular fracture of distal radius.Methods A total of 78 patients with intra-articular fracture of distal radius were divided into Kirschner wire group (42 cases) and plate group (36 cases).The Kirschner wire group was treated by Kirschner wire combined with external fixator and the plate group by locking compression plate.The bone mineral density,healing rate,joint function and incidence of complications were compared between the two groups.Results There was no significant difference in healing rate,healing time,radius shorten distance and bone mineral density between the two groups (P>0.05).For C1 and C2 type patients,the joint function between Kirschner wire group plate group had no significant difference (P>0.05).For C3 type patients,the joint function score in Kirschner wire group was significantly lower than that of the plate group,and the difference between the two groups was statistically significant (P<0.05).The incidence of complications between Kirschner wire group and plate group was not significantly different (P>0.05).Conclusion For C3 type fracture patients,the clinical efficiency of external fixator combined with Kirschner wire is better than locking compression plate.
