滑膜软骨瘤病—临床,X线与病理表现的关系

报告23例滑膜软骨瘤病,膝关节11例,髋关节6例,肘关节4例,踝关节2例。常见的临床表现是疼痛、肿块和活动受限。骨软骨小体典型X线征象是中间密度低、周围密度高的环状阴影。软骨体钙化程度不一,开始可为软骨,最后全部骨化。手术治疗19例:游离体摘除14例,游离体摘除加滑膜切除5例。随访12例,平均随访时间6年,仅l例复发。认为第三期病变不必行滑膜切除术。

Synovial Chondromatosis.Correlation between Radiological,Pathological and Clinical Manifestations

Twenty-three cases of synovial chondromatosis (11 knee, 6 hip, 4 elbow and 2 ankle) are reported. The most common symptoms and signs were pain, mass and limitation of joint movement. The typical roentgenologic appearance of osteocartilaginous bodies is a round shadow with central low density in contrast to higher density in the periphery. The degree of calcification in cartilaginous body is at variance ranging from simply cartilaginous,at the one extreme to completely ossified at the other extreme. Nineteen cases were treated surgically, with removal of loose bodies merely in 14 cases, synovectomy plus loose bodies removal in 5. Twelve cases were followed up from 1.5 to 12.5 years, with an average of 6 years. Recurrence was noted only in one case. It is the authors' opinion that synovectomy is not indicated if the disease is well less than the third stage.
