| 摘 要： | 目的探讨乳腺增生病在重度乳腺增生及癌前病变阶段中医辨证分型与彩色超声多普勒（CDFI）、高频钼钯BI-RADS分级的相关性;分析影像学在中医辨证及癌前病变筛选中的应用,建立健全对乳腺增生病尤其重度乳腺增生及癌前病变阶段中医微观辨证指标。方法针对局限性腺体增厚、临床手检不能检出的亚临床病灶的高危人群301例进行辨证分型,观察血流动力学指标、BI-RADS分级、病理组织学分级等微观指标能否客观反映出乳腺癌癌前病变阶段中医辨证特点和病机演变规律。结果病理进展到乳腺癌癌前病变阶段,中医证候分布集中于痰瘀互结（偏肝郁气滞、偏冲任失调）型,与肝郁气滞、冲任失调型比较：CDFI示Vmax、RI增高,BI-RADS分级Ⅲ、Ⅳ级比例增高。结论 BI-RADS分级及血流动力学指标Vmax、RI改变符合乳腺癌癌前病变的病机由肝郁气滞、冲任失调日久痰瘀互结的演变进程,可反映乳腺病变的恶性倾向,可作为乳腺增生病中医辨证的微观指标和中医治疗评价标准。
