PFNA内固定治疗高龄患者股骨转子间骨折35例疗效分析

目的评价应用股骨近端抗旋髓内钉（PFNA）治疗股骨转子间骨折的临床疗效。方法2008年2月至2009年1月，采用PFNA钉闭合复位内固定治疗35例高龄股骨粗隆间骨折患者，评价其临床效果。结果本组35例，2例因脑血管意外等疾病死亡，2例失访，其余31例经12～23个月随访（平均18．2个月），骨折均获得一期愈合，骨折平均愈合时间为8～16周，平均12．8周。其中2例患者由于螺旋刀片退出致大腿部疼痛。根据Harris髋关节评分，优良率达83．9％。结论PFN闭合复位内固定治疗股骨粗隆间骨折，具有手术创伤小、固定可靠、应力分布分散、适合早期功能锻炼、愈合效果好等优点，是高龄股骨粗隆间骨折较理想的内固定物。

Analysis of therapeutic efficacies on Proximal Femoral Nail Anti-rotation(PFNA)in the treatment of 35 elderly patients with intertrochanteric fractures of femur

Objective To analyze the theropeutic effects of treatments with proximal femoral nail antirotation(PFNA)in senile people with intertrochanteric fractures of femur.Methods From February 2008 to January 2009.35 senile patient with femoral intertrochanteric fracture received fixation of PFNA were analyzed retrospectively.Results Two cases died of cardiovascular and cerebrovascular disease and two cases were not followed up.The 31 cases were followed up from 12 to 23 month after operation(average18.2 months).Bone union,range from 8 weeks to 16 weeks(average 12.8 weeks),was obtained in the 31 cases.2 case of sliding out with screw blade,which lead thigh pain,were observed.According to the Harris criterion.The excellent rate of hip function was 87.8%.Conclusion PFNA fixation technique is a good method for treatment of elderly patients with femoral intertrochanteric fracture.
