腹腔镜手术、局部冲洗及药物联合治疗未破裂型输卵管妊娠

目的：观察未破裂型输卵管妊娠的治疗效果及持续性异位妊娠（ＰＥＰ）的发生率。方法：采用腹腔镜下输卵管妊娠物清除结合局部冲洗和局部注射氨甲喋呤的方法治疗３０例要求保留生育的未破裂型输卵管妊娠患者，并观察术后血人绒毛膜促进性腺激素的变化。结果：３０例患者均顺利完成手术，术后第１天血ＨＣＧ值较术前降低〉６０％的占８６．７％；无１例ＰＥＰ发生。结论：腹腔镜手术联合局部冲洗及药物治疗是治疗输卵管妊娠的一种较好的方法。此方法可减少ＰＥＰ的发生；术后第１天血ＨＣＧ值可作为预测ＰＥＰ的参考指标。

Laparoscopic Surgery Combining with Local Douche and Injection of MTX in Treatment of Non-rupture Tubal Pregnancy

Objective: To evaluate the effect of the treatment in the patients suffering from non-rupture tubal pregnancy and the incidence of persistent ectopic pregnancy (PEP) in these patients. Methods: Thirty patients who desired to conserve function of fertility, suffeing from non-rupture tubal pregnancy were treated by laparoscopic surgery. The laparoscopic surgery consisited of extraction of embryonic mass combining with local douche and injection of methothexate. The serum HCG of these patients was examined postoperatively. Results: The operations were totally successful in the 30 cases. On the first postoperative day, the HCG levels of 86.7% patients decreased over 60% than their preoperative data. All of the patients were safe and no occurance of PEP. Conclusions: It is a better method od choice in treatment of non-rupture tubal pregnancy. This method may prevent from occurance of PEP. The HCG level on the first postoperative day may predict the occurance of PEP.
