胺碘酮与普罗帕酮治疗老年阵发性心房颤动疗效与安全性研究

目的 探讨胺碘酮与普罗帕酮治疗老年阵发性心房颤动的疗效与安全性.方法 106例阵发性房颤患者随机分成胺碘酮治疗组和普罗帕酮治疗组,分别为54例和52例,随访1年,观察两组治疗前后房颤复发情况及左房内径的变化.结果 胺碘酮治疗组预防老年阵发性房颤复发3m的总有效率为88.9％,6m的总有效率为77.8％,9m的总有效率为74.1％,12 m的总有效率为70.4％;普罗帕酮治疗组3m的总有效率为73.1％,6m的总有效率为63.5％,9m的总有效率为57.7％,12m的总有效率为53.8％;胺碘酮治疗组左房内径在治疗前后发生了变化（P＜0.05）;两组药物长期应用不良反应均较为少见.结论 胺碘酮在老年阵发性房颤的治疗比普罗帕酮相对有效,两组药物合理使用不良反应均较低.

The effect and safety of amiodarone and propafenone in treatment of elderly patients with paroxysmal atrial fibrillation

Objective To explore the effect and safety of amiodarone and propafenone in treatment of elderly patients with paroxysmal atrial fibrillation （AF）. Method One hundred and six patients with paroxysmal atrial fibrillation were randomly divided into amiodarone group and propafenone group, 54 and 52 cases respectively. The AF recurrent rate and change of inner diameter of left atrium were compared before and after one year treatment with 2 groups. Result The total effective rate of amiodarone group was 88.9 percent three months, 77.8 percent six months, 74.1 percent nine months, 70.4 percent one year. And the total effective rate of propafenone group was 73.1 percent three months, 63.5 percent six months, 57.7 percent nine months, 53.8 percent one year. Left atrial diameter in amiodarone group was changed after treatment （P 〈 0.05）. Side effects with two group were rare in both groups. Conclusion The treatment of amiodarone in elderly patients with paroxysmal atrial fibrillation is relatively more effective than that of propafenone, and few side effect was observed in both groups.
