江苏省南通地区乙型肝炎病毒基因分型与临床相关性

目的了解江苏省南通地区乙型肝炎病毒(HBV)基因型分布状况及其临床相关性。方法选择南通地区HBV-DNA阳性的乙型肝炎病毒感染者300例,其中乙型肝炎表面抗原携带者(ASC)28例,慢性乙型肝炎(CHB)189例,重型肝炎(CSH)26例,肝硬化(LC)30例,肝细胞癌(HCC)27例,采用多对型特异性引物巢式PCR法检测HBV基因型。结果300份血清标本中B型89例(29.7%),C型196例(65.3%),BC混合型15例(5.0%);C基因型在LC组、HCC组中的比例显著高于ASC组和CHB组(P<0.05)。结论南通地区HBV基因型可能以B、C型为主,C型为优势基因型并与肝硬化、肝癌的发生相关。

Genotyping of hepatitis B virus and its clinical significance

Objective To investigate the distribution of hepatitis B virus genotypes in Nantong and its clinical significance.Methods Serum samples from 300 HBV-DNA positive patients with chronic HBV infection,including 28 asymptomatic HBV carriers（ASC）,189 chronic hepatitis B（CHB）,26 chronic severe hepatitis（CSH）,30 liver cirrhosis（LC）,and 27 hepatocellular carcinoma（HCC） patients were collected and determined for HBV genotypes by PCR with genotype-specific primers.Results Of the 300 samples,89（29.7%） were genotype B and 196（65.3%） were genotype C,15（5.0%） were coinfected by genotype B ＆ C.Genotype C was more prevalent in the LC and HCC patients than in the ASC and CHB patients（P〈0.05）.Conclusions The dominant genotype of HBV in Nantong is genotype C;Genotype C is associated with the development of severe liver diseases.
