房室传导不典型文氏现象的心电图特点及临床意义

目的探讨房室传导不典型文氏现象的心电图特点及临床意义。方法回顾性分析9例(其中男6例,女3例,年龄19～81岁)房室传导不典型文氏现象的临床资料。结果9例房室传导不典型文氏现象病例中,1例为单纯房室阻滞,1例交替性文氏传导,2例非交替性文氏传导,5例房室结双径路。其中4例为病理性阻滞伴严重器质性心脏病,其他为迷走高敏所致生理性阻滞。结论年轻患者不典型文氏型房室阻滞者以迷走高敏所致功能性阻滞多见,有器质性心脏病者发生不典型文氏现象时,心电图表现复杂并常与其他心律失常共存。

The electrocardiogram characteristics and clinical meaning of untypical Wenckebach''''s phenomenon in atrioventricular conduction

Objective to discuss the electrocardiogram characteristics and clinical meaning of untypical Wenckebach's phenomenon in atrioventricular conduction. Methods Retrospectively analyzed 9 cases (6 male, 3 female, age from 19 to 81)with untypical Wenckebach's phenomenon in atrioventricular conduction. Results Among the 9 cases, one was simple atrioventricular conduction block, one were alternative Wenckebach's conduction, two were not alternative Wenckebach's conduction, 5 were dual atrioventricular nodal pathways. 4 of them were Pathologic block with serious structural heart diseases, the others were physiological block caused by high sensitive vagus nerve. Conclusions most young patients with untypical Wenckebach's conduction are functional caused by high sensitive vagus nerve. When patients with structural heart diseases have untypical Wenckebach's phenomenon, their electrocardiograms are complex and coexist with other types of arrhythmias.
