磁共振在诊断强直性脊柱炎相关骶髂关节病变中的应用

目的探讨X线平片、CT及MRI等影像检查方式对于强直性脊柱炎相关骶髂关节病变的诊断价值。方法对23例临床怀疑为强直性脊柱炎患者的x线平片、CT及MRI检查结果进行分析。结果23例患者中x线平片检查确诊病变的7例共9侧；CT检查明确11例18侧骶髂关节病变；MRI检查23例患者均表现为异常，特别是X线平片及CT不能显示的骨髓水肿、滑膜病变、脂肪沉积、肌腱附着点炎、被摸炎及关节软骨破坏。结论MRI能发现X线平片及CT不能显示的强直性脊柱炎的早期病变表现，为临床怀疑强直性脊柱炎病变相关性骶髂关节炎的首选检查方法。

The Diagnosis of Relative Sacroiliac Joint Lesions of Ankylosing Spondylitis on MRI

Objective To compare the diagnostic value of image examination methods （including X-ray plain film, CT and MRI） in relative sacroiliac joint lesions of ankylosing spondylitis. Methods Perform analysis and statistics to X-ray plain film, CT and MR examination results of 23 patients that were clinically suspected as ankylosing spondylitis. Results Among the 23 patients, 9 sides of 7 cases were confirmed as lesion by X-ray plain film examination; 18 sides of 11 cases as sacroliac joint lesion by CT examination; 23 patients all showed abnormal by MRI examination, especially those that cannot be indicated by X-ray plain film and CT, such as bone marrow edema, synovial membrane lesion, fiat de- position, tendon enthesitis, capsule inflammation and joint cartilage destruction. Conclusion MRI can find early lesion manifestation of AS that cannot be indicated by X-ray plain film or CT and it can be the first examination choice for rela- tive sacroiliac arthritis of AS lesion that were clinically suspected.
