口腔颌面损伤442例临床分析及治疗心得

目的回顾性分析颌面部损伤患者的临床资料,对患者性别、年龄、致伤原因、合并损伤及治疗方法进行临床流行病学总结。方法对辽宁医学院附属第一医院口腔科,2007年9月至2011年12月收治的442例颌面部骨折患者的病例资料进行统计和分析。结果 442例颌面部骨折患者男女比例为2.81：1,21～40岁为发生的高峰年龄段,占71.0%。骨折以5、8、9月份多发。交通事故在致伤原因中居首位,占55.7%。颌面部骨折类型以下颌骨骨折发生率最高,占43.2%。伴有神经损伤者219例,其中以眶下神经损伤发生率最高,占47.94%。合并损伤以四肢损伤最为多见,占合并损伤患者的37.86%,颅脑损伤居第2位,占26.70%。另有颌面部外伤伴寰枢关节半脱位2例。颌面骨折治疗以坚固内固定为主要方式,占83.60%。结论颌面部骨折发病率逐年上升,交通事故为主要致伤原因,发病人群以青壮年为主,坚固内固定为主要治疗方式。

Retrospective analysis of 442 patients with facial trauma

【Objective】 Retrospective analysis of patients with maxillofacial injuries clinical data on patient sex,age,cause of injury,combined injuries and treatment summary of the clinical epidemiology.【Methods】 First Affiliated Hospital of Liaoning Medical College of Stomatology,from September 2007 to December 2011 treated 442 patients with maxillofacial fractures in patients with clinical data were statistically analyzed.【Results】 442 cases of maxillofacial fractures in patients with male to female ratio was 2.81：1. 21 to 40 years for the occurrence of the peak age,accounting for 71.0%.Multiple fractures of the 5,8,9 month.Causes of injury in traffic accidents in the first place,accounting for 55.7%.The following types of maxillofacial fractures highest incidence of mandibular fractures,accounting for 43.2%.219 cases associated with nerve injury,in which the highest incidence of infraorbital nerve injury,accounting for 47.94%.Combined damage to the limbs the most common injury,accounting for 37.86% of patients with combined injury,brain injury ranks No.2,accounting for 26.70%.Another maxillofacial trauma with atlantoaxial subluxation in 2 patients.Maxillofacial fractures in the main method of rigid internal fixation,accounting for 83.60%.【Conclusion】 The incidence of maxillofacial fractures increased year after year,traffic accidents as the main cause of injury,the incidence of people are young,rigid internal fixation as the main treatment.
