髌骨内推治疗膝骨关节炎疗效观察

目的观察髌骨内推治疗膝骨关节炎(KOA)的临床疗效。方法将90例KOA患者随机分为2组,对照组45例予盐酸氨基葡萄糖胶囊治疗,治疗组45例在对照组治疗基础上应用髌骨内推治疗。2组均治疗1个月。比较2组治疗后1、3个月疗效;观察2组治疗前后双下肢肌肉围度、膝关节髌骨压痛评分及国际膝关节文献委员会膝关节评估表(IKDC)评分变化。结果治疗组治疗后1个月治愈率37.78%,总有效率84.44%;对照组治疗后1个月治愈率17.78%,总有效率68.89%。治疗组治疗后3个月治愈率62.22%,总有效率95.56%;对照组治疗后3个月治愈率28.89%,总有效率77.78%。2组治疗后3个月治愈率、总有效率均高于本组治疗后1个月(P0.05);治疗组治疗后1、3个月治愈率、总有效率均高于对照组治疗同期(P0.05)。2组治疗前、治疗后1个月患侧下肢肌肉围度均较本组健侧肌肉围度减少(P0.05)。2组治疗后1、3个月患侧肌肉围度均较本组治疗前增加(P0.05)。治疗组治疗后1、3个月患侧下肢肌肉围度均高于对照组治疗同期(P0.05)。2组治疗后3个月患侧下肢肌肉围度均较本组治疗后1个月增加(P0.05)。2组治疗后膝关节髌骨压痛评分低于本组治疗前(P0.05),治疗后3个月膝关节髌骨压痛评分低于治疗后1个月(P0.05);治疗组治疗后膝关节髌骨压痛评分低于对照组治疗同期(P0.05)。2组治疗后IKDC评分均高于本组治疗前(P0.05),治疗后3个月IKDC评分高于治疗后1个月(P0.05);治疗组治疗后IKDC评分高于对照组治疗同期(P0.05)。结论髌骨内推治疗KOA疗效确切,可以改善患者关节疼痛及延缓退化变形,无不良反应,宜长期坚持。

Clinical effects of patellar inward pushing on the treatment of knee osteoarthritis

Objective To observe the clinical effects of patellar inward pushing on the treatment of knee osteoarthritis (KOA).Methods 90 KOA patients were randomly divided into two groups.45 cases in control group were treated by glucosamine hydrochloride capsules.45 cases in treatment group were treated by patellar inward pushing on the basis of the control group.The treatment course was 1month in two groups.The curative effects after treatment 1month, 3 months were compared in two groups.The changes of double lower limbs muscles circumference, knee joint patella tenderness score and the international knee documentation committee knee uation form (IKDC) score before and after treatment were observed in two groups.Results The cure rate (37.78%) and the total effective rate (84.44%) after treatment 1 month in treatment group were superior to those in control group (17.78%, 68.89%, P<0.05).The cure rate (62.22%) and the total effective rate (95.56%) after treatment 3 months in treatment group were superior to those in control group (28.89%, 77.78%, P<0.05).The cure rates and the total effective rates after treatment 3 months were higher than those after treatment 1 month in two groups (P<0.05).The cure rats and total effective ratses after treatment 1 month,3 months in treatment group were higher than those in control group at the same period(P<0.05).The muscle circumferences of ill side before and after treatment 1 month were less than the healthy side in two groups (P<0.05).The muscle circumferences of ill side after treatment 1 month and 3 months were increased in two groups (P<0.05), and which in treatment group was higher than that in control group at the same period (P<0.05).The muscle circumferences of ill side after treatment 3 months were increased compared with that after treatment 1 month in two groups respectively (P<0.05).The scores of knee joint patella tenderness after treatment were lower than before treatment in two groups (P<0.05), and which after treatment 3 months were lower than after treatment 1month (P<0.05).The score of knee joint patella tenderness after treatment in treatment group was lower than those in control group at the same period (P<0.05).The scores of IKDC after treatment were higher than before treatment in two groups (P<0.05), and which after treatment 3 months were higher than after treatment 1month (P<0.05).The score of IKDC after treatment in treatment group was higher than that in control group at the same period (P<0.05).Conclusion Patellar inward pushing has exact effects on the treatment of KOA, can improve the patient's joint pain, delay the degradation and deformation of joint, with no adverse reactions, it should be insisted for a long time.
