骨通贴膏配合手法复位联合小夹板外固定术治疗桡骨远端骨折疗效观察

目的观察骨通贴膏配合手法复位联合小夹板外固定术治疗桡骨远端骨折的疗效。方法将155例桡骨远端骨折患者随机分为3组,对照1组51例行手法复位联合小夹板外固定术治疗,对照2组49例行无药物贴膏配合手法复位联合小夹板外固定术治疗,治疗组55例行骨通贴膏配合手法复位联合小夹板外固定术治疗。7 d为1个疗程,共治疗4个疗程。比较3组术前及术后1个月腕关节疼痛及肿胀程度变化,术后第6个月腕关节活动度及功能变化、骨折愈合率、并发症发生率。结果 3组术后1个月腕关节疼痛VAS评分及肿胀程度评分均较本组治疗前明显下降,且治疗组低于对照1、2组,比较差异均有统计学意义(P0.05),对照1组与对照2组间比较差异无统计学意义(P0.05)。术后第6个月,治疗组腕关节背伸角、掌曲角、桡偏角、尺偏角、旋前角明显小于对照1、2组,腕关节功能优良率高于对照1、2组,比较差异均有统计学意义(P0.05);对照1组与对照2组之间比较差异无统计学意义(P0.05)。3组间骨折愈合率及并发症发生率比较差异均无统计学意义(P0.05)。结论骨通贴膏配合手法复位联合小夹板外固定术对桡骨远端骨折有良好的治疗效果,能明显减轻术后腕关节疼痛、肿胀,恢复腕关节活动度及功能。

Observation of Gutong plaster,manual reduction and small splint external fixation on the treatment of distal radius fracture

Objective To observe the clinical effects of Gutong plaster, manual reduction and small splint external fixation on the treatment of distal radius fracture.Methods 155 patients with distal radius fracture were randomly divided into three groups.51 cases in control group 1 were only treated by manual reduction combined with small splint fixation.49 cases in control group 2 were treated by manual reduction, small splint fixation and no drug paste.55 cases in treatment group were treated by Gutong plaster, manual reduction and small splint external fixation.A treatment course was 7 d, continuously treatment for 4 courses.The degree changes of pain and limb swelling before and after operation 1 month were compared in three groups.The changes of wrist joint mobility and functional, the rate of fracture healing and the incidence of complications after operation 6 months were compared in three groups.Results The scores of Visual analogue scale (VAS) and swelling degree after treatment 1 month were reduced obviously in three groups, and which in treatment group were lower than those in the control group 1 and control group 2 with statistical difference (P<0.05), and there was no statistical difference between control group 1 and control group 2 (P>0.05).The wrist back hade, palm curved angle, radial angle, ulnar inclination and pronation angle after 6 months treatment in treatment group were significantly smaller than those in the control group 1 and control group 2.The excellent and good rate of wrist joint function was higher than that in control group 1 and and control group 2, with statistical difference (P<0.05), and there was no statistical difference between control group 1 and control group 2 (P>0.05).There was no statistical difference on the rate of fracture healing and the incidence of complications among the three groups (P>0.05).Conclusion Gutong plaster, manual reduction and small splint external fixation have a good therapeutic effect on the treatment of distal radius fracture, can significantly reduce postoperative swelling, pain, recovery mobility and function of wrist joint.
