全髋关节置换术在强直性脊柱炎髋关节强直治疗中的临床效果及安全性研究

目的评价全髋关节置换术治疗强直性脊柱炎髋关节强直的临床疗效及安全性。方法对我院收治的41例（52个髋关节）强直性脊柱炎髋关节强直患者进行全髋关节置换术治疗，对手术前后的髋关节疼痛情况、Harris评分及髋关节功能进行比较分析，并对手术的安全性进行评估。结果采用Harris评分系统对术后临床疗效进行评价，其中优25髋，良16髋，中11髋，总有效率为100％。术后患者的疼痛程度显著低于术前（P〈0．05），而术后的Harris评分及髋关节活动度显著高于术前（P〈0．05）。对41例患者进行12-24个月的随访，患者术后均能生活自理，治疗效果均比较满意，未出现感染、脱位等近期并发症。结论人工全髋关节置换术用于治疗强直性脊柱炎髋关节强直能够显著的提高髋关节的活动度，减轻患者疼痛，改善患者的生活质量，且并发症少，治疗效果值得肯定。

The clinical efficacy and safety of total hip arthroplasty in the treatment of ankylosing spondylitis hip an- kylosis

Objective To study the clinical efficacy and safety of total hip arthroplasty treating ankylo- sing spondylitis hip ankylosis. Methods 41 patients （52 cases of hips） with ankylosing spondylitis hip ankylo- sis reeeived total hip arthroplasty. Hip pain eonditions, Harris scores,hip functions of preoperative and postop- erative were compared and analyzed, and the safety of surgery evaluated. Results The postoperative clinical efficacy were evaluated by Harris scoring system. Harris scores showed that there were 25 eases of excellent, 16 eases of good,ll cases of general,the total efficiency were 100%. Postoperative pain intensity was significantly lower than the preoperative （ P 〈 0. 05 ）. Postoperative Harris seores and hip activity were significantly higher than the preoperative（ P 〈 0. 05 ）. The patients were followed up 12 ~ 24 months, all the 41 patients were able to take care of themselves, the treatment effects were satisfied, no term compheatlons such as infeetion, disloeation occurred. Conclusion Total hip arthroplasty treating ankylosing spondylitis hip ankylosis can improve the activity of the hip significantly, reduce pain, improve the quality of life of patients, reduce complications, the efficacy is worthy of recognition.
