紧急子宫切除术治疗难治性产后出血临床探讨

目的探讨紧急子宫切除在难治性产后出血中的临床价值及如何降低产科紧急子宫切除率。方法回顾性的分析31例难治性产后出血被迫行紧急子宫切除治疗的临床疗效，并与宫腔纱布填塞治疗30例进行比较。分析紧急子宫切除治疗难治性产后出血的临床价值。结果子宫切除组31例，抢救成功31例，成功率100％；输血量（1760．3±48．21）ml。宫腔填塞组30例，抢救成功29例，成功率98％；输血量（2298．1±55．7）ml。结论规范紧急子宫切除在难治性产后出血中的临床应用，有助于提升患者的救治成功率，降低产科紧急子宫切除率。

The Clinical Probe into the Treatment for Intractable Postpartum Hemorrhage with Emergency Hysterectomy

Objective A probe into the treatment for intractable postpartum hemorrhage as well as the way of reducing the emergency hysterectomy rate. Methods Analyzing the curative effects of 31 cases of in- tractable postpartum hemorrhage treated with emergency hysterectomy. Then making a comparison with 30 ran- domly selected cases treated with filling in the uterine cavity with gauze. Thus analyzing the clinical values of treating intractable postpartum hemorrhage with emergency hysterectomy. Results 31 cases treated with hyster- ectomy had a 100% success rate, the amount of blood transfusion was （ 1760. 3 ~ 48.21 ） ml, 30 cases treated with filling in the uterine cavity with gauze had a 98% success rate, the amount of blood transfusion was （2298.1 ± 55.7） ml. Conclusion Standard emergency hysterectomy for treating intractable postpartum hemor- rhage not only can contribute to the success rate of the patients＇ rescue and treatment, but also can reduce the e- mergency hysterectomy rate.
