栗锦迁教授辨治心悸学术思想简析

栗锦迁教授对心悸的治疗本于《内经》、法于张仲景,总结出该病虚实两分、兼顾五脏、全面考虑、突出主证、灵活辨治等观点。对该病的辨证分型总体上分为以虚为主证型和以实为主的证型,以虚为主的证型包括阳虚气弱和气血两虚两大类。正虚致悸(阳气不足、阴血不足)和邪实致悸(火热致悸、水饮及痰饮致悸、瘀血致悸)。以实为主的证型包括痰火扰心、痰浊阻闭、心血瘀阻、七情郁结、突受外惊。在该病的辨证治疗上,强调要兼顾虚实两端,脏器亏虚需首辨阴阳气血,标实需分清水饮、痰浊、瘀血,以及其两两组合或三者共见中的主要矛盾,在不同的阶段,根据不同的邪实特征灵活组方,基于虚则补之,实则泻之,而功补结合做到攻不伤正,补不滞邪。
