阴式子宫肌瘤剔除术的应用体会

目的：探讨阴式子宫肌瘤剔除术的临床应用价值及安全性。方法：选择2009年2月~2011年2月在本院接受子宫肌瘤剔除术的患者180例,包括接受阴式子宫肌瘤术（阴式组）90例和接受腹式子宫肌瘤术（腹式组）90例。比较两组患者手术时间、出血量、术后排气时间、术后住院时间以及术后病率。结果：两组平均手术时间（62.78±26.28）minvs（68.28±25.31）min]、术中平均出血量（202.68±56.43）mlvs（210.12±56.83）ml]比较差异无统计学意义（P〉0.05）;而阴式组术后平均排气时间（23.61±5.63）h]、术后平均住院天数（6.15±0.32）d]、术后病率（2.22%）均显著短（低）于腹式组（34.65±6.63）h、（7.12±0.33）d、13.33%],差异具有统计学意义（P〈0.01或0.001）。结论：阴式子宫肌瘤剔除术是一种安全有效、创伤小、术后排气时间、住院时间短、术后病率低的手术方法,值得临床推广。

The application experience of vaginal myomectomy

Objective： To study the clinical effect, feasibility and safety of vaginal myomectomy. Methods： From February 2009 to February 2011, 90 cases of patients underwent vaginal myomectomy （VM group） and the other 90 patients underwent abdominal myomectomy（AM group）. Operative time, blood loss, postoperative anal exhaust time, postoperative hospital stay and postoperative morbidity rate were compared between the two groups. Results： Compared to the VM group, operative time （62.78±26.28） min vs （68.28±25.31） mini, blood loss （202.68±56.43） ml vs （210.12±56.83） ml] in the LAP group were no significantly difference （P〉0.05）; Compared to the VM group, postoperative anal exhaust time（23.61 ±5.63） h, （34.65±6.63） h], postoperative hospital stay （6.15±0.32） d, （7.12±0.33） d] and postoperative morbidity rate （2.22%, 13.33%） in the LAP group were significantly shorter （P〈0.05）. Conclusion： Vaginal myomectomy is a safe, effective, less trauma, shorter postoperative hospital stay, and lower postoperative morbidity, worthy of clinical application.
