吻合器痔上黏膜环切术(PPH)176手术体会

目的探讨吻合器痔上黏膜环切术(PPH)提高手术疗效、减少并发症的技巧。方法回顾性分析176例Ⅲ、Ⅳ度混合痔患者的手术资料,总结手术操作的要点。结果平均手术时间22(15～55)min,切除组织宽度平均3.0(2.0～3.7)cm,本组全部患者切割吻合均一次成功,无大出血、大便失禁、吻合口漏、直肠周围组织损伤等并发症,术后平均住院3(2～7)d,术后随访4周~12个月,无复发及吻合口狭窄等。结论充分认识影响PPH手术效果的因素,注重其细节的改进,可减少并发症,提高手术疗效。

Surgical Experience of Hemorrhoid Stapling Mucosal Resection (PPH) on 176 Cases

Objective To investigate the hemorrhoid stapling mucosal resection （PPH） to improve surgery and reduce complications skills. Methods Retrospective analysis of 176 eases Ⅲ、Ⅳ degree of mixed hemorrhoid surgery patients information, summarize the main points of surgical operations. Result The average operation time was 22 （15-55） min, removal of tissue width of the average 3.0 （2.0-3.7） cm, all patients in this group were consistent with a successful cutting, no bleeding, fecal incontinence, anastomotie leakage, damage to surrounding tissue and other concurrent rectal disease, mean postoperative hospital stay 3 （2~7） d, were followed up for 4 weeks to 12 months, no recurrence and anastomotic stenosis. Conclusion Fully understand the effect of the factors affecting PPH surgery, focusing on the details of the improvements, reduce complications and improve surgical treatment.
