肱骨近端三部和四部分骨折的外科治疗

目的：寻找一种治疗肱骨近端三部或四部分骨折的新方法．方法：以肱二头肌长头腱为标志，切开复位、Ｔ形钢板内固定，同时修复撕裂的肩袖和关节囊，术后早期康复治疗．结果：７例随访３个月～４年，平均２．５年，患侧肩关节功能优良率达８５．７％．结论：Ｔ形钢板内固定治疗肱骨近段三部或四部分骨折具有内固定牢靠、术后不需外固定、早期功能锻炼的优点，有助于肩关节功能恢复．

Surgical Treatment of Three and Four Part Fractures Proximal to Humerus

Aim: To seek for a new method to treat three and four part fractures proximal to humerus. Methods: Three and four part fractures proximal to humerus were reducted openly and fixed internally with T shaped plate by the mark of long head of biceps muscle of arm, and the lacerated rotator cuff or articular capsule was repaired at the same time. Postoperative convalescent treatments were carried out early. Results: Seven patients were followed up from 3 months to 4 years, averaged 2.5 years. The rate of ideal function of the disordered shoulder joints after operation was 85.7%. Conclusion: Internal fixation with T shaped plate to treat severe fractures proximal to humerus has the advantages of rigid internal fixation, no need for external fixation after operation, earlier convalescent training and being helpful to recover functions of the disordered shoulder.
