颅脑外伤85例凝血指标变化分析

目的:探讨急性颅脑损伤患者受伤后部分凝血指标变化对判断损伤程度及预后的价值。方法:对85例急性颅脑损伤病人测定其伤后24h内的各项凝血指标:PT(凝血酶原时间)、INR(凝血酶原时间国际正常化比值)、APTT(活化部分凝血活酶时间)、FBG(纤维蛋白酶原)、TT(凝血酶时间)、Plt(血小板计数),并结合临床格拉斯哥评分(GSC)分型及格拉斯哥预后评分(GOS)预后分级,进行组间比较并与健康对照组比较。结果:患者损伤程度越重,PT、INR、APTT水平越高,FBG水平越低,且与预后存在一定相关性。结论:凝血指标测定可作为临床应用的监测指标,对判断颅脑损伤程度及预后具有临床参考价值。

The analysis of the blood coagulation disturbance in eighty-five brain injury

Objective ：To investigate the characteristics of blood coagulation and fibrinolysin in acute brain injury and evaluate their effects on the severity and prognosis. Methods ：Eighty five patients were divided into three groups according to the disease severity of acute brain injury. To detect the levels of PT,INR,APTT, FBG,TT and Pit of patients within 24h after their admission according to GCS and GOS,and compare the levels among the three groups as well as the healthy group. Results：PT ,INR and APTT were all abnormal in all groups of acute brain injuries. The more the severity of brain injury , the higher the levels of PT ,INR and APTT,the lower the level of FBG,related to the prognosis. Conclusion： The patients with acute brain injuries have the disturbances of blood coagulation and fibrinolysin , which are related to the severity and prognosis of brain injury. The changes could be an index to judge the severity and prognosis of patients suffered from cranioeerebral injury.
