认知行为干预疗法对鼻咽癌放疗病人生存质量的影响

目的通过临床实验验证“音乐治疗加放松内心意象法”这种改良的认知行为干预方法的效果,探讨本疗法能否提高鼻咽癌放疗病人的生存质量。方法将98例初治鼻咽癌病人随机分为两组,实验组48例,接受放疗和“音乐治疗加放松内心意象法”干预治疗;对照组50例,单纯放疗,两组于治疗前及治疗结束后均应用SCL－90症状自评量表进行测试。结果治疗前98例病人SCL－90的9个因子分均值高于国内常模,经t检验(P<0.01)有非常显著性差异;治疗结束后两组SCL－90的9个因子分均值也有显著性差异(P<0.05)。结论鼻咽癌放疗病人存在着严重的心理障碍,采用“音乐治疗加放松内心意象法”这种改良的认知行为干预治疗可以减轻病人的心理障碍,提高生存质量。

Effect of cognitive behavioral intervention therapy on quality of life in nasopharyngeal carcinoma during radiotherapy

Objective To test the effect of"musicotherapy and relaxation inner image"on quality of life(QOL)of nasopharyngeal car-cinoma during radiotherapy.Method98cases of nasopharyngeal carcinom a patients were randomly divided int o two groups:The study group(n =48)received both radiotherapy and"musicotherapy and relaxation inner image";The control group(n =50)received only radiotherapy.The two groups received a psychopathologic al symptom survey before radiothera py by using the Symptoms Checklist -90scale(SCL -90).At the end of radiotherapy,all the patients were survey once again by the same measure of SCL -90.Result The results showed that the scores of9factors in the SCL -90were significantly higher than that of regular domestic model in all of the patients before radioitherapy(P<0.01).It was found that there were significant difference b etween the two groups in most of the 9f actors at the end of radioherapy(P<0.05).Conclusion This study indicats the existence of severe psy chological disturbance in all of the patients.The"musicotherapy and relaxation inner image"can reduce psychological disturbance and impr ove the life quality of the patients.
