延续性护理对喉癌全喉切除术老年患者生活质量的影响

目的研究喉癌全喉切除术老年患者术后生活质量,探讨延续性护理对患者生活质量的影响。方法 2012年6月—2013年5月收集92例喉癌全喉切除老年患者,随机分为研究组和对照组,研究组采用延续性护理干预,对照组采用常规的健康教育和住院护理,比较3个月前后患者的生活质量。结果干预前两组患者生活质量差异无统计学意义（P〉0.05）,干预后研究组的疼痛（79.89±17.18）、娱乐（83.15±21.12）、吞咽（83.15±16.71）、讲话（85.33±22.72）、心情（82.07±18.00）、焦虑（89.67±13.52）得分均显著高于对照组的疼痛（67.39±22.28）、娱乐（70.11±34.00）、吞咽（72.83±22.87）、讲话（69.57±31.14）、心情（70.65±28.04）、焦虑（78.26±26.67）,两组比较,差异均有统计学意义（均P〈0.05）。结论延续性护理能有效地改善喉癌患者全喉切除手术后的生活质量,提高患者的自我护理能力。

Effect of continued nursing on quality of life in elder laryngeal carcinoma patients undergoing total laryngectomy

[Objective]To study the quality of life in elder laryngeal carcinoma patients undergoing total laryngectomy,explore the effect of the continued nursing on patients＇ quality of life. [Methods]92 elder laryngeal carcinoma patients undergoing total laryngectomy were collected from June 2012 to May 2013,and randomly divided into the study group and the control group. The study group was given the continued nursing,and the control group received the routine health education and nursing care. The patient＇s quality of life before and after three months of the intervention was compared.[Results]Before the intervention,there was no significant difference in quality of life between two groups（ P〈0. 05）. After the intervention,the scores of pain（ 79. 89 ± 17. 18）,entertainment（ 83. 15 ± 21. 12）,ingurgitation（ 83. 15 ± 16. 71）,speech（ 85. 33 ± 22. 72）,mood（ 82. 07 ± 18. 00） and anxiety（ 89. 67 ± 13. 52） in the study group were higher than those in the control group [pain（ 67. 39 ± 22. 28）,entertainment（ 70.11 ±34.00）,ingurgitation（ 72. 83 ± 22. 87）,speech（ 69. 57 ± 31. 14）,mood（ 70. 65 ± 28. 04） and anxiety（ 78. 26 ±26. 67） ],and the differences were significant（ P〈0. 05）.[Conclusion]The continued nursing can effectively improve the quality of life in elder laryngeal carcinoma patients undergoing total laryngectomy,and enhance their self-care skills.
