超敏C反应蛋白与尘肺肺感染合并肺心病病情的相关性分析

摘    要:
目的探讨血清中超敏C反应蛋白(hs-CRP)与尘肺肺感染合并肺心病及病情的关系。方法收集2011年11月—2012年8月在国家安全生产监督管理总局职业安全卫生研究中心职业病科住院的经临床诊断为尘肺病合并肺部感染患者150例,其中合并肺心病的42例,比较单纯尘肺肺感染组和尘肺肺感染合并肺心病组及肺心病不同心功能水平组间血清中hs-CRP浓度的差异。结果尘肺肺感染合并肺心病组与尘肺肺感染组血清中hs-CRP浓度分别为(30.656±24.608)mg/L和(7.625±6.833)mg/L,经t检验,二者差异有统计学意义(t=8.923,P〈0.01)。尘肺肺感染与合并肺心病1~4级心功能组的血清中hs-CRP浓度分别为(7.625±6.833)、(13.920±12.732)、(35.060±18.166)、(47.686±17.181)和(64.980±28.682)mg/L,随肺心病心功能级别增加而增高,差异有统计学意义(F=66.978,P〈0.01)。结论血清中hs-CRP浓度与尘肺肺感染合并肺心病有关,与肺心病心功能级别相关,心功能越差血清hs-CRP浓度越高。

Abstract:
[ Objective] To investigate the relationship between C-reactive protein( hs-CRP) and pulmonary heart disease with pneu- moconiosis induced pulmonary infection. [Methods] A total of 150 cases of inpatients with pneumoconiosis and pulmonary infection, including 42 cases with pulmonary heart disease, diagnosed clinically from November 2011 - August 2012 in occupational disease department of Occupational Safety and Health Research Center of State Administration of Work Safety were collected for comparison of serum concentrations of hs-CRP among patients of simple pulmonary infection group, pulmonary heart disease with pulmonary infection group and pulmonary heart disease group. [ Results] The serum concentrations of hs-CRP of pulmonary heart disease with pulmonary infection group and simple pulmonary infection group were {30. 656 ± 24. 608) mg/L and 17. 625 ± 6. 833 ) rag/L, with significant difference ( t = 8. 923, P 〈 0. 01 ) The serum concentrations of hs-CRP of patients with pneumoconiosis induced pulmonary infection and pulmonary heart disease of grade 1- 4 were ( 7. 625 ± 6. 833 I mg/L, ( 13. 920 ± 12. 732 ) mg/L, (35. 060± 18. 166 ) mg/L, (47. 686 ± 17.181 ) mg/L, ( 64. 980± 28. 682 )mg/L, respectively. It increased with the elevating grade of pulmonary heart disease, with significant difference (F = 66. 978, P 〈 0.01 }. [ Conclusion ] The serum concentrations of hs-CRP is associated with pulmonary heart disease and pneumoconiosis induced pulmonary infection, and the grade of pulmonary heart disease. The poor cardiac function the patients have, the higher the hs-CRP concentration is.
