北京市顺义区2012年健康人群脊髓灰质炎抗体水平

摘    要:目的了解北京市顺义区健康人群脊髓灰质炎(简称脊灰)抗体水平,为卫生部门已制定的免疫策略提供依据。方法随机选取顺义区10个乡街的6个月以上健康人群共243人,采集其静脉血以微量中和试验法检测脊灰抗体。结果243份血清标本中脊髓灰质炎Ⅰ、Ⅱ、Ⅲ型病毒抗体的阳性率分别为97.94%、98.35%、97.53%,抗体几何平均滴度倒数(GMRT)分别为98.74、89.61、69.54,各型间GMRT有差异(F=4.574,P〈0.05),表现为Ⅰ〉Ⅱ〉Ⅲ型;户籍、性别、年龄、职业、脊灰疫苗免疫史均不是影响Ⅰ、Ⅱ、Ⅲ型抗体水平阳性率的因素,但年龄、免疫史是影响各型抗体滴度的因素,年龄越大抗体滴度越低,免疫次数越多抗体滴度越大(P〈0.05),免疫史〈4次和≥4次Ⅰ、Ⅱ、Ⅲ型病毒的抗体GMRT差异有统计学意义(tⅠ=5.63、tⅡ=4.02、tⅢ:6.69,P均〈0.05)。结论顺义区健康人群脊灰抗体水平较高,对脊灰形成免疫屏障。随着年龄的增大,抗体滴度下降,完成全程免疫即4剂次免疫人群抗体滴度更高,脊髓灰质炎疫苗全程免疫很必要。

Abstract: Objective ] To know the polio antibody level among healthy population in Shunyi District of Beijing, provide a scientific evidence for health departments to formulate the immunization strategies. Methods ] 243 healthy people over 6 months were ran-domly selected from 10 towns/streets in Shunyi District, and the venous blood samples were collected to detect the polio antibody by micro neutralization test. Results] Among 243 serum samples, the positive rate of antibodies against poliovirus type I, H and III was 97.94%, 98.35% and 97.53% respectively, and geometric mean reciprocal titer ( GMRT) was 98.74, 89.61 and 69.54, respectively. There were significant differences in GMRT among poliovirus type Ⅰ, Ⅱ and Ⅲ ( F = 4.574, P 〈 0.05 ) , which were in sequence of type Ⅰ 〉 type Ⅱ 〉 type Ⅲ. The household registration, sex, age, career and polio immunization history were not the factors that irffluenced the positive rates of antibodies levels. However, the age and immunization history were the influencing factors of GMRT. GMRT decreased with increased age, and it raised with increased frequency of poliomyelitis vaccination. There were sig-nificant differences in GMRT of poliovirus type Ⅰ, Ⅱ and Ⅲ between people who received poliomyelitis vaccination less than 4 doses and people who received poliomyelitis vaccination equal or greater than 4 doses t ( type Ⅰ ) . 5.63, t ( type Ⅱ ) = 4.02, t type ( Ⅲ ) = 6.69, all P 〈 0.051. Conclusion] The polio antibody level is high among healthy population in Shunyi District, and a stable immunization barrier has been established. GMRT decreases with increased age, while the GMRT is higher in people who have received 4 doses of poliomyelitis vaccination, and the full dose immunization of poliomyelitis is necessary.
