不同类型冠心病患者外周血树突状细胞分型及功能研究

目的：研究不同类型冠心病患者外周循环中树突状细胞（DCs）数量和功能的变化，探讨DCs与冠脉粥样硬化斑块从稳定到“易损”的关系．方法：80入选者被分为稳定型心绞痛组（SAP）（17例）、肌钙蛋白阴性不稳定型心绞痛组UAP（CTNI-），15例]、肌钙蛋白阳性不稳定心绞痛组UAP（CTNI＋），15例]、急性心肌梗死组（AMI）（15例），冠状动脉造影阴性对照组（CTL）（18例）．流式细胞4色分析法检测DCs及亚型的比例；流式细胞仪及酶联免疫吸附法（ELISA）检查DCs功能．结果：与CTL组相比，SAP患者外周血DCs占外周血白细胞的比例和数量无显著变化；UAP（CTNI-）患者DCs比例和数量显著增高；AMI患者DCs比例和数量均减少；UAP（CTNI＋）患者DCs比例降低．培养第7日，UAP（CTNI-），UAP（CTNI＋），AMI组CD83表达高于CTL，SAP组，UAP（CTNI＋）、AMI组显著高于UAP（CTNI-）组（P〈0．01）．培养第7日，在DCs数量为2×10^8／L时，UAP（CTNI-），UAP（CTNI＋），AMI组DCs刺激T细胞增殖的能力显著高于CTL，SAP组（P〈0．01）；UAP（CTNI＋），AMI组显著高于UAP（CTNI-）组（P〈0．01）．混合淋巴细胞反应上清液中IL-12，IL-10，IFN—α．浓度：UAP（CTNI-），UAP（CTNI＋），AMI显著高于CTL，SAP组；UAP（CTNI＋）、AMI组显著高于UAP（CTNI-）组（P〈0．01），AMI组较UAP（CTNI＋）组有升高趋势．结论：检测冠心病外周血DCs数量、比例及功能可以一定程度反应冠心病的进展．
