健康管理在社区老年慢性病防治中的应用与效果评价

目的 探讨健康管理对社区老年慢性病防治的影响. 方法 对福田区益田社区181例老年慢性病人进行为期1年的健康调查、评估、干预,并进行健康管理前后效果评价. 结果 181例老年慢性病人生活方式得到明显优化,体育锻炼习惯有显著改善（P＜0.05）,膳食结构更加合理,低盐饮食有明显下降（P＜0.05）;同时通过一年的健康管理后,血糖、血压、血脂达标率均较健康管理前明显提高,慢性病与非药物治疗知晓率也明显提高（P＜0.05）.其它遵医及服药依从性也有明显改善. 结论 对社区老年慢性病人进行健康管理可控制和减少老年人由生活方式导致的慢性病,维护和提高老年人的健康水平,提高生活质量.

Application and Effect Assessment of Health Management in Prevention and Control of Community Senile Chronic Diseases

Objective To explore the influence of health management on prevention and control of community senile chronic diseases.Methods We conducted health survey,assessment and intervention among 181 elderly patients with chronic diseases in Yitian Community,Futian District for a one-year period.And the intervention effect was evaluated before and after the health management.Results The lifestyle and physical exercises of 181 elderly patients were improved significantly （P＜0.05） ; moreover,the dietary structure and low-salt diet were more reasonable （P ＜ 0.05）.After one-year health management,the percentages of patients with blood glucose,blco d pressure,and blood lipids reaching to the therapeutic standards and the awareness rates of chronic diseases and non-drug treatment were all increased significantly （P＜ 0.05）.Behaviors related to following doctor＇ s instructions and medication adherence were also improved significantly （P ＜ 0.05）.Conclusions Health management on community elderly patients with chronic diseases can control and reduce chronic diseases caused by lifestyle,maintain and improve the health status and quality of life of the elderly.
