高龄肺癌患者全肺切除术临床研究

目的探讨高龄肺癌患者行全肺切除术的可行性及围手术期处理。方法回顾性分析34例高龄肺癌患者行全肺切除术后出现心肺并发症的危险因素，总结围手术期处理经验。结果本组34例高龄肺癌患者术后心律失常发生率为55．88％，患者术前合并心血管病史、心电图异常、肺功能异常、术中出血量多、手术时间长及术后血清钾偏低与术后心律失常发生相关。呼吸系统并发症发生率为11．77％，无手术死亡。结论全肺切除术是外科治疗高龄肺癌的有效方法之一，手术指征的正确选择，积极有效的围手术期治疗及护理，可有效减少术后心肺并发症，降低死亡率。

Retrospective analysis of pneumonectomy resection surgery treatment in senile patients with lung cancer 34 cases

Objective To discuss the feasibility and perioperative treatment of pulmonary resection surgery treatment for senile lung cancer patients. Methods We retrospectively analyzed the risk factors of cardiopulmonary complications in 34 cases lung cancer with senile patients who underwent pneumonectomy resection surgery treatment , summed up the experience of perioperative treatment. Results The perioperative arrhythmias occurrence rate was 55.88%. Cardiovas- cular disease history, arrhythmia history, cardiac and pulmonary dysfunction before operation, more blood loss, lower postoperative serum potassium , operation time were the high risk factors of perioperative arrhythmia. Conclusion Pneumonectomy resection surgery treatment in elderly lung cancer patients was feasible. The correct choice of surgical indications, perioperative positive and effective treatment and nursing, could be effective in reducing postoperative car- diopulmonary complications and mortality.
