腹腔镜胆囊切除术治疗急性胆囊炎疗效观察

目的探讨急性胆囊炎患者应用腹腔镜进行胆囊切除治疗的临床疗效。方法选择2010年8月至2013年1月遂宁市中医院收治的急性胆囊炎患者48例，设为腹腔镜组，应用腹腔镜进行胆囊切除术。另选择同期收治的急性胆囊炎患者47例，设为对照组，应用传统的开腹胆囊切除术治疗。观察两组术中、术后的各指标的变化。结果腹腔镜组手术时间（70．5±30．3）min、术中出血量（50．7±10．4）ml、下床活动时间（11．0±5．0）h、胃肠活动时间（1．5±0．6）d、平均住院时间（5．0±2．6）d等指标均明显少于对照组（P〈0．05）；腹腔镜组患者术后并发症发生率为8．00％，低于对照组的16．98％（P〈0．05）。结论腹腔镜胆囊切除术治疗急性胆囊炎临床疗效显著、创伤小、患者痛苦小、术后恢复快，术者应严格掌握手术适应症，熟练掌握腹腔镜的基本操作和技巧。

Efficacy of laparoscopic cholecystectomy for acute cholecystitis

Objective To investigate the clinical efficacy of the laparoscopic cholecystectomy for patients with acute cho- lecystitis. Methods August 2010 to January 2013 Suining TCM Hospital admitted 48 patients with acute cholecystitis, set the laparoscopic group, application laparoseopic cholecystectomy. Another choice earlier admitted 47 patients with a- cute cholecystitis,as controls,the application of traditional open cholecystectomy. Were observed postoperative changes in each index. Results Laparoscopic operative time （70. 5 ± 30. 3） min,blood loss （50. 7 ± 10. 4）ml,ambulation time （ 11.0 ± 5.0） h, gastrointestinal activity time （ 1.5 ± 0. 6 ） d, the average length of stay （ 5.0± 2. 6 ） d and other indicators are significantly less than the control group （P 〈 0. 05 ） ;Laparoscopic group postoperative complication rate was 8. 00%, lower than the control group, 16. 98% （P 〈 0. 05 ）. Conclusion Laparoscopic cholecystectomy for acute eholecystitis has significant clinical effect, trauma, little pain, rapid postoperative recovery, patients should be strictly controlled surgi- cal indications, master the basic operations and laparoscopic techniques.
