摘    要:
目的探讨血液学指标在孕妇β-地贫筛查中的价值。方法采用血细胞、血红蛋白分析结合血清铁蛋白水平对妊娠妇女进行13-地贫筛查,筛查阳性者进行β-珠蛋白基因分析,用ROC曲线分析MCV、MCH和HbA2最佳cutoff值,评价其在β-地贫筛查中的价值。结果MCV、MCH和HbA2的ROC曲线下面积依次为0.992、0.986和0.987,最佳cutoff值分别为75.1n,25.3pg和3.8%,灵敏度和特异性分别为96.9%和97.2%、96.9%和96.8%、98.7%和99.6%%。结论以ROC曲线分析得到的MCV、MCH和HbA:最佳cutoff值对β-地贫具有较好的诊断价值。

Values of analyzing haematological indicators with ROC curve for screening thalassemia in pregnant women

Abstract:
Objective: To investigate the value of haematological indicators screening for β -thalassemia in pregnant women. Methods : Hematology, hemoglobin and serum ferritin anlaysis were carried out among pregnant women, and β - globin gene anlaysis were carried out among them with the high - risk of β - thalassemia. Optima cutoff values of MCH, MCV and HbA2 were analyzed by ROC curve in screening of β - thalassaemia heterozygote. Results : The area under ROC curve of MCH, MCV and HbA2 in order was 0. 992, 0. 986 and 0. 987, and the optimal cutoff values were 75. 113, 25.3pg and 3.8% respectively. The sensitivity of them were 96. 9%, 96. 9% and 98.7%, and the specificity of each were 97.2%, 96. 8% and 99. 6% respectively. Conclusion: The optimal cutoff values of MCH, MCV and HbA2 might have good diagnostic application for screening β - thalassaemia heterozygote.
