应用超声心动图对缺血性二尖瓣反流发生机制的研究

目的通过对二尖瓣形态学改变与二尖瓣反流程度相关性的研究来探讨缺血性二尖瓣反流发生机制。方法缺血性二尖瓣反流患者31例和20例正常对照。超声心动图观察指标：二尖瓣关闭点心尖方向、前后方向位移，前后叶的关闭角度和活动度及二尖瓣反流量。结果二尖瓣关闭点心尖方向、后向位移增加和前叶受牵拉增强是缺血性二尖瓣反流的独立决定因素。结论缺血性二尖瓣反流是由于二尖瓣前后叶同时受到增强牵拉作用的结果。

Echocardiographic Investigation of Mechanism in Ischemic Mitral Regurgitation

Objective The relationship between leaflet configurations and persistent ischemic mitral regurgitation(MR) were investigated to recognize the mechanism of ischemic MR.Methods In 31 consecutive patients with ischemic MR and 20 controls,the posterior and apical displacement of the leaflet coaptation,the anterior leaflet and posterior leaflet tethering angles together with mobility,and the MR grade were quantified with echocardiography.Results The increases in apical and posterior displacement of the coaptation and in anterior mitral leaflet AML tethering were the independent determinants of MR.Conclusions Both leaflets tethering simultaneously are the major determinants of ischemic MR.
