社区成人获得性支原体肺炎92例临床特点分析

[目的]探讨成人社区获得性支原体肺炎（CAP）的临床特点.[方法]回顾性分析2008年1月至2011年1月本院确诊的CAP 患者92例的临床资料.[结果]CAP最常见症状有发热、顽固性咳嗽,有皮疹、鼻衄等肺外损害,发病初期所有患者肝功能无损害,周围血象中白细胞均〈10×109/L,4例合并胸腔积液,X线胸片及CT表现以片状渗出及间质改变为主,CT表现以中上肺野为好发部位,阿奇霉素治疗疗效满意.[结论]CAP临床表现多样化,伴有肺外器官的损害,血清特异性抗体阳性仍是诊断支原体感染的主要手段.

Analysis of Clinical Features of 46 Adult Patients with Community Acquired Mycoplasma

[Objective] To explore the clinical features of community acquired Mycoplasma pneumoniae （CAP） infection in adults. [Methods] Clinical data of 92 CAP patients confirmed in our hospital from Jan. 2008 to Jan. 2011 were analyzed retrospectively. [Results] The most common symptoms of CAP were fever, refractory cough and extra pulmonary damages such as skin rash and epistaxis. In the initial stage of the disease, liver function of all patients was normal and white blood cell count was lower than 10 × 109/L. There were 4 patients complicated with pleural effusion. The main manifestations of X-ray and CT scan were slabby infiltration and interstitial change. CT scan showed that upper field of lung was often affected. Azithromycin for the treatment of CAP had satisfactory efficacy. [Conclusion] Clinical manifestation of CAP is diversifica- tion and complicated with extra pulmonary damages. Specific seropositive antibody is still main way to diag- nose Mycoplasma infection.
