Risk Factors of Lacunar Infarction in Hypertensive Patients

Aim:To analysis the risk factors of lacunar stroke in essential hypertensive patients. Methods:Seventy five patients with EH were classified into two groups based on with lacunar infarction( n =48) and without lacunar infarction( n =27) by MRI test. Oral glucose tolerance tests,lipid analyses,platelet aggregation,blood viscosity,uric acid,microalbuminuria,tPA,PAI 1 and 24 hours ABPM were studied. Logistic regression analyses was used to find the independent risk factors of lacunar infarction. Results:High density lipoprotein were significantly decreased in the lacunar infarction compared with the group without. Microalbuminuria,PAI 1,the mean systolic blood pressure and the maximum systolic blood pressure levels were significantly higher in the lacunar infarction group .In the lacunar infarction group,the maximum insulin and 3 hour insulin area(area under the plasma insulin concentration curve) during a 75 g oral glouse tolerance test were significantly increased.Logistic regression analysiss showed microalbuminuria and PAI 1 may be independent risk factors of lacunar infarction. Conclusions:Beside the hypertension,the HDL c,microalbumin uria,insulin curve under area,and PAI 1 may be contribute the risk factors in lacunar infarction of hypertension patients.
