88例脑梗死患者数字减影全脑血管造影分析

目的观察脑梗死患者数字减影全脑血管造影（DSA）检查所显示的血管狭窄或闭塞的比例及其分布。方法 88例脑梗死患者均经头颅CT或MRI确诊并排除出血。上述患者全部进行选择性颈部和颅内血管造影。观察血管狭窄部位、受累血管数目以及血管狭窄形态并判断血管狭窄程度。结果 88例脑梗死中血管造影显示69例（78.41%）有血管狭窄或闭塞,其中58例有颈动脉系统受累,29例有椎基底动脉系统受累,锁骨下动脉受累8例。其中38例同时有多部位受累。颈动脉系统中以颈内动脉和大脑中动脉受累最为常见,椎基底动脉系统受累的以椎动脉开口处最为常见。19例血管造影正常。结论脑梗死患者DSA显示血管病变以颈内动脉及椎动脉开口部位病变为主。DSA是一项安全有效的诊断脑血管病变的工具。

Analysis of clinical application of digital subtraction angiography for 88 cases of cerebral infarction patients

Objective To observe the proportion and its distribution of what the brain digital subtraction angiography(DSA)can show vascular stenosis or occlusion of cerebral infarction patients.Methods 88 cases of cerebral infarction patients with digital subtraction angiography(DSA)examination were diagnosed by cranial CT or MRI and excluded hemorrhage.To observed the involvement area and the number of vascular stenosis and therefore determine the degree of stenosis.Results Among 88 cases of cerebral infarction patients,69 cases(78.41%)of all patients of cerebral angiography showed vascular stenosis or occlusion and 19 cases showed normal angiography,in which 58 patients had carotid artery system involvement,and 29 patients had vertebrobasilar arterial system involvement,another 8 had subclavian artery involvement.At the same time,38 cases of which were involved.The internal carotid artery and middle cerebral artery involvement of carotid artery system and vertebral artery openings of the vertebrobasilar arterial system is easy involved.Conclusion DSA is safe,simple and sensitive method for diagnosis of cerebrovascular lesion.Cartoid artery(especially internal carotid artery)lesion play important role in patients with cerebral infarction.
