原因不明性复发性流产91例主动免疫治疗效果分析

目的探讨原因不明性复发性流产(URSA)行主动免疫治疗的疗效及免疫细胞含量的变化特征。方法应用荧光染色和流式细胞技术,分别对2011年1月至2012年8月在安徽医科大学第一附属医院产前诊断中心行淋巴细胞主动免疫治疗的91例URSA患者治疗前后淋巴细胞坏死率及CD3、CD4、CD8及NK细胞比率进行测定;并随访其妊娠结局。结果 79例成功妊娠至12周胎盘形成以后,12例在孕12周以前再次流产,成功率86.81%;治疗后CD3、CD4和NK细胞比例明显下降,CD8及淋巴细胞坏死率显著上升,差异具有统计学意义(P0.05)。结论淋巴细胞主动免疫治疗对于URSA具有显著的疗效,NK细胞的降低有利于妊娠成功。

Lymphocyte active immunotherapy in 91 cases with unexplained recurrent spontaneous abortion

Objective To investigate the effectiveness of lymphocyte active immunotherapy in patients with unexplained recurrent spontaneous abortion and the changes of immune cells before and after the treatment as well as the impact on subsequent pregnancy. Methods Fluorescence staining and flow cytometry were used to determine cell nercrosis of lymphocytes CD3, CD4, CD8 and NK cell percentage in 91 unexplained recurrent spontaneous abortion patients between Jan 2011 and Aug 2012 in the First Affiliated Hospital of Anhui Medical University; meanwhile the patients were followed up. Results Among the patients, 79 cases were pregnant successfully to 12 weeks, but 11 had a miscarriage before 12 weeks, and the success rate of active immunotherapy was 86.81%. CD3, C D4 and NK cells declined, and the necrosis rate and CD8 sub - type ratio increased after treatment （ P 〈 0. 05 ; P 〈 0. 05 ）. The difference was statistically significant. Conclusion Lymphocyte active immunotherapy is effective for unexplained recurrent spontaneous abortion, and the reduction of NK cells may be helpful for the development of pregnancy.
