腮腺良性肿瘤手术中面神经的定位及处理

目的 探讨腮腺良性肿瘤术中面神经的定位和处理.方法 对28例腮腺良性肿瘤手术进行回顾性分析,所有病例均保留面神经,并在手术中进行精细解剖和主动保护.结果 3例出现术后暂时性面瘫,于1～3周内自行恢复正常,其余病例术后神经功能良好.随访1～18个月,无肿瘤复发,无永久性面瘫和其他严重并发症发生,手术效果满意.结论 腮腺区手术与面神经关系密切,正确处理面神经将有利于减少术后并发症的发生.

Applied anatomy and treatment of facial nerve in the operation of benign parotid tumors

Objective To explore the applied anatomy and treatment of the facial nerve in the operation of benign parotid tumors. Methods Twenty-eight cases with benign parotid tumors treated by surgery were analyzed retrospectively. In all eases, the facial nerve and its branches were dissected cautiously, protected consciously and proserved completely. Results Temporary postoperative facial paralysis of marginal mandibular branch was found in three eases and recovered naturally in 1~3 weeks after the operation. The period of follow--up varied from 1~18 months, and the final results were satisfactory. There were no tumor recurrenee, and no permanent facial paralysis and serious complications were observed. Conclusion The anatomic relationship between the facial nerve and parotid region was close. Suitable treatment of facial nerve might be associated with less eomplieation.
