MR磁敏感成像诊断缺血性中风的应用价值

摘    要:
目的探讨磁敏感加权成像技术(susceptibility weighted imaging,SWI)在缺血性脑中风的诊断价值。方法采用Siemens公司Magnetom Espree 1.5T磁共振成像系统.应用SWI和其它常规技术对126例缺血性脑中风患者进行对比研究。常规MR成像技术包括SE序列T1和T2、T2梯度回波、T2FLAIR、DWI。结果(1)SWI显示中脑以上缺血性病灶能力优于常规,T2WI和T2梯度回波,并可明确缺血性脑中风类型。(2)诊断静脉窦血栓形成和监测缺血性脑梗塞转化出血性脑梗塞有突出优势。(3)在腔隙梗塞为主要表现的病人,其他序列检测出大多数腔隙软化病灶比SWI敏感;但SWI显示非脑软化灶、微出血灶优于常规序列。(4)SWI可显示缺血性病灶或梗塞灶内的血管变化和侧枝血管的建立。结论SWI在缺血性脑中风的诊断、监测和决策临床治疗方案有重要意义。

Abstract:
Objective To study the value of susceptibility weighted imaging in the diagnosis of ischemic stroke. Methods 126 cases with ischemic stroke were examined with Siemens 1.ST Magnetom Espree. Both conventional MR technology and SWI were performed. Conventional technology included SE T1/T2, T2*WI, T2FLAIR and DWI. Results (1) SWI is better than conventional T2WI and T2*WI in displaying ischemic lesions above the level of middle brain and it is helpful to diagnose ischemic stroke type. (2) SWI has great advantages in the diagnosis of cerebral venous sinus thrombosis and monitoring the transformation from ischemic infarction to hemorrhage stroke. (3) The sensitivity of conventional examination is better than that of SWI in detecting cerebral old lunacular infarct, while SWI is better than conventional method in displaying micro-bleeds and fresh lesions of lunacular infarct. (4) SWI could display the blood vessels change and the collateral vessels formation in ischemic or infarct lesion. Conclusion SWI has vital significance in diagnosing, monitoring ischemic stroke and is helpful to choose clinical treatment.
