Genetic variation of hepatitis B virus P gene in treatment of hepatitis B by adefovir dipivoxil

Objective To analyse the situations of HBV P gene variation in treatment of hepatitis B by adefovir dipivoxil.Methods 106 chronic hepatitis B patients who were treated by adefovir dipivoxil more than one year were detected for P gene variation,and the levels of HBV DNA and ALT were compared.Results There were 60 cases with rt181 gene variation single,15 cases with rt236 gene variation single,and 9 cases with the combination variation,and the total detect rate was 79.25％ (84 cases).The HBV DNA levels in the cases with rt181 gene variation and the cases with the combination variation were significant higher than those in non-variation cases ( P ＜ 0.05).There was no statistical significance between cases with rt181 gene variation and non-variation cases ( P ＞ 0.05).The HBV DNA levels of the cases with the combination variation was significant higher than those in other cases ( F =16.273,P ＜ 0.05).Compared to non-variation cases,the ALT levels of the cases with gene variation were significantly increased ( F =57.999,P ＜0.05).The ALT levels of the cases the combination variation at rt181 was significant higher than that in other variation cases (P＜0.05).After 3 months treatment of lamivudine,there was no significantly decreased about the HBV DNA levels in 12 cases with rt181 gene variation ( F =2.736,P ＞ 0.05),and the HBV DNA levels in15 cases with rt236 gene variation decreased significantly ( F =33.618,P ＜ 0.05).Conclusions Adefovir dipivoxil can induce the P gene variation,importantly,the patients with different variation had different levels of HBV DNA.
