腹腔镜与开腹胆囊切除术患者围手术期单核细胞趋化蛋白-1表达的变化

目的:比较腹腔镜胆囊切除术与开腹胆囊手术患者围手术期单核细胞趋化蛋白-1(MCP-1)表达的变化。方法:选取择期行胆囊切除术患者40例,随机分为腹腔镜胆囊切除术组(L组)和开腹胆囊切除术组(O组)两组,每组各20例。分别在入室后(T1)、切皮前(T2)、手术进行30 min后(T3)、手术结束后患者清醒时(T4),在外周静脉采血4 mL,测定血浆单核细胞趋化蛋白-1浓度。结果:两组麻醉诱导前MCP-1浓度差异无统计学意义(P>0.05),L组手术结束时MCP-1浓度与T1时比较浓度明显升高,在T4时点与O组比较差异有统计学意义(P<0.05)。结论:对MCP-1表达的影响,开腹组要优于腹腔镜组,这可能与CO2气腹及手术时间较短有关。

Effect of laparoscopic and open cholecystectomy on expression of monocyte chemotactic protein-1 during perioperative period

Objective：To investigate the effect of laparoscopic and open cholecystectomy on expression of monocyte chemotactic protein-1 during perioperative period.Methods：Forty patients undergoing elective cholecystectomy were divided randomly into 2 groups：laparoscopic eholecystectomy group（group L） and open cholecystectomy group（group O）.Venous blood samples were taken before anesthesia（T1）,before operation（T2）,30 min later after operation（T3）,and at the end of operation（T4）,respectively.Monocyte chemotactic protein-1 levels（MCP-1）were detected by enzyme linked immunosorbent assay in plasma.Results：MCP-1 levels increased at the end of operation in group L.The levels of MCP-1 was significantly lower than those in group O at the end of operation（P0.05）.Conclusions：Compared to open surgery,laparoscopic cholecystectomy has obvious depressive effects on the expression of MCP-1.It is possibly attributed to by CO2 pneumoperitoneum and shorter surgery time.
