MALT淋巴瘤临床病理分析

目的:观察黏膜相关淋巴组织结外边缘区B细胞淋巴瘤(MALT淋巴瘤)的临床和病理特点。方法:回顾性分析海南医学院附属医院病理科2000年1月~2010年8月29例MALT淋巴瘤的临床病理资料,通过HE染色和免疫组化染色进行病理组织学观察。结果:29例MALT淋巴瘤中,发生于胃肠道共25例,占86.21%,临床主要表现为胃肠道疾病非特异性症状,其中13例胃MALT淋巴瘤中,HP感染10例,感染率为76.92%;发生于眼眶3例,甲状腺1例。镜下肿瘤组织主要由小淋巴细胞组成,部分病例伴有少量大细胞和淋巴上皮病变。瘤细胞表达B淋巴细胞免疫标记CD20和CD79a,CD5和CD10无表达。结论:胃肠道是MALT淋巴瘤的好发部位,不同部位的MALT淋巴瘤具有共同的细胞形态和免疫表型特征。

Pathological analysis of 29 cases with MALT lymphoma

Objective：To observe the clinical and pathological features of extranodal marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue（MALT lymphoma）.Methods：Pathological and clinical data of 29 MALT lymphoma patients that was sent to Department of Pathology,the Affiliated Hospital of Hainan Medical University during January,2000 and August,2010 were retrospectively analyzed by H﹠E staining and immunohistochemical staining assay.Results：Of the 29 cases,25 cases occurred at gastrointestinal tract,accounting for 78.13%,with non-specific clinical manifestations.There were 13 cases of gastric MALT lymphoma,of which 10 were complicated with Helicobacter pylori infection（10/13,76.92%）.Other 3 of the left cases were orbit MALT lymphoma and 1 was thyroid MALT lymphoma.Microscopically,the tumor was mainly composed of small lymphocytes,in some cases accompanied by a few large cells and lymphoepithelial lesions.Expression of immunostaining CD20 and CD79a were positive in these tumor cells while that of CD5 and CD10 were negative.Conclusions：Gastrointestinal is the predilection site for MALT lymphoma,which shows the same morphological and immunophenotypic features regardless of where the tumor occurred.
