纤维蛋白原、脂蛋白a、尿酸与冠状动脉病变程度相关性研究

摘    要:目的 探讨血浆纤维蛋白原、脂蛋白a、尿酸与冠状动脉病变程度相关性。方法 选择行冠状动脉造影的患者146例,根据造影的结果分为冠脉狭窄组78例与非冠脉狭窄组68例,采用Claues法测定血浆纤维蛋白原(Fig)水平,采用尿酸酶比色法测定尿酸(UA),脂蛋白a(Lpa)采用免疫比浊法测定,同时检测血清胆固醇(1℃)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)水平。根据狭窄累及冠状动脉的数分为1支、2支和3支病变组,观察其与Fig、Lps、UA的关系。结果 冠脉狭窄组血浆Fig异、血清Lpa及血清UA明显高于非冠脉狭窄组(HDL-C低于冠脉狭窄组),P〈0.05,血浆Fig、血清Lpa及血清UA随冠状动脉病变支数增加而升高(P〈0.05)。多元线形回归分析HDL与冠脉狭窄积分呈负相关,年龄、吸烟、舒张压(DBP)、UA、Fig、1℃与冠脉狭窄积分呈正相关。结论 Fig、Lps、UA与冠状动脉病变程度密切相关,可能对冠心病(CHD)发病存在一定影响。

Abstract:Objective The aim of present study was to investigate the respective influence of fibrinogen,lipoprotein(a),uric acid on severity of coronary artery stenosis.Methods 146 cases who had undergone diagnostic coronary angiography were divided into two groups according to the effect of coronary artery angiography.One group is the patients with coronary artery stenosis(n=78),and another group is those without coronary artery stenosis(n=68).The serum fibrinogen,lipoprotein(a) and uric acid were detected in 146 patients.Two groups patients' plasma lipids levels were measured.According angiography,patients were divided into single-,double-and triple-vessel groups.We explored the relationship among fibrinogen,LP(a),UA with the severity of coronary artery stenosis.Results The level of fibrinogen,LP(a)and UA in serum of subjects with coronary stenosis were significant higher than subjects without coronary stenosis,and this difference was significant between the group included subjects with one coronary atery stenosis,and the group included subjects with three coronary artery stenosis.Multivatiate linear stepwise regression anlysis show the age,smoking,diastolic blood pressure,the serum levels of Fig,LP(a) and UA of the subjects were posistively correlated,whereas,the serum level of HDL was negatively with coronary artery stenosis score.Conclusion There is a close correlation between Fig,LP(a),UA and the severity of coronary artery stenosis.
