摘    要:
目的:提高对腕管综合征(CTS)的诊断水平。方法:应用针极肌电图与神经传导速度(NCV)对30例46侧腕管综合征(CTS)患者进行了检测和分析。结果:正中神经腕管以下支配肌针极肌电图见失神经支配电位而不累及其他神经支配肌;腕管以下正中神经运动传导速度(MCV)、MCV潜伏期及感觉传导速度(SCV)均有不同程度的减慢,MCV潜伏期与针极肌电图的异常率比值极其相近,之间存在显著相关性。不经过腕管的尺神经与腕管以上的正中神经传导速度(NCV)均正常。结论:针极肌电图对观察CTS的损害范围与程度有重要意义,NCV不同部位和分节段的检查可以排除其他周围神经病变而确诊CTS。

Abstract:
Objective: To improve the diagnostic accuracy of carpal tube syndrome (CTS). Method : 46 carpal tubes of 30 patients suffering from CTS were detected by needle pole electromyogram and their nerve conductive velocity (NCV) was examined. Results: Dnervated potentials were observed on the electromyograms of muscle controled by the middle nerve far from carpal tube, while not those muscles done by other nerves. The motor conductive velocity (MCV), the latent period of MCV and sensor conductive velocity (SCV) slowed down more or less. The abnormal rates of the electromyograms and the latent period of MCV were almost the same, and there was an obvious correlation between them. NCV of ulnar nerve which does not move through the carpal tube and of middle nerve close to the carpal tube were normal. Conclusion: The needle pole electromypgram is important in detecting the extent of CTS and CTS can be diagnosed by examining NCV in different positions.
