急性脑梗死患者凝血及外周血白细胞水平与梗死面积的关系

目的探讨急性脑梗死(ACI)患者凝血和外周血白细胞水平与梗死面积的关系。方法对176例ACI住院患者根据CT或MRI扫描结果将梗死灶分为小、中、大3组,小梗死组72例,中梗死组66例,大梗死组38例,分别测定其凝血和外周血白细胞水平,将梗死面积与凝血和外周血白细胞水平作相关性分析。结果血浆纤维蛋白原(Fbg)、凝血酶原时间(PT)、白细胞总数(WBC)、中性粒细胞百分比(NE)与不同梗死面积组之间差异呈统计学意义(P<0.05),不同梗死面积与Fbg、WBC、NE均呈显著正相关(r值分别为0.46、0.57、0.35,P<0.05),而与PT、APTT、TT无相关性(r值分别为0.20、0.45、-0.41,P>0.05)。多元回归分析显示,Fbg、WBC和脑梗死的严重程度存在独立相关性。结论ACI患者的Fbg、WBC与脑梗死的程度相关,推测其可作为脑梗死严重程度的参考指标。

The Relationship Between Coagulation, Peripheral WBC and Infarction Areas in Patients with Acute Cerebral Infarction

Objective To explore the relationships between coagulation, Peripheral WBC and infarction areas in patients with acute cerebral infarction(ACI).Methods 176 patients with ACI were recruited in the study.Different area of cerebral infarction were divided three groups(small infarct(72 patients),moderate infarct(66 patients)and large infarct(38 patients))based on CT or MRI.Different area of cerebral infarction and coagulation,WBC were analysed.Results The levels of Fbg,PT,WBC and NE had significant difference with increasement of degree of severity of ACI(P<0.05).There was significant positive correlation among the level of Fbg,WBC,NE and different area of cerebral infarction(r=0.46,0.57,0.35,P<0.05),but there was no significant correlation among them and PT,APTT,TT(r=0.20,0.45,-0.41,P>0.05).Multiple linear regression showed that Fbg and WBC had significant correlation to ACI.Conclusion The levels of Fbg and WBC are related to the severity of cerebral infarction,which have important prognostic implications in patients with ACI.
